医保总额预付制进入医改视野

    在医改问题上，许多医疗专家建议的医保“总额预付制度”将有望实现。昨天国新办举行新闻发布会，卫生部部长陈竺在会后接受记者采访时表示，此前被称做新医改方案重要倾向之一的“总额预付制度”，卫生部正在积极研究中。

    随着卫生部昨天对此进行的首次表态，这种政府财政医保资金定额支付给医疗结构的最终期许将有可能出现在医改新方案中，并得以实现。此前有专家透露，卫生部规划财务司正在对该制度下的收费价格体系进行研究。

    所谓“总额预付制”，是由医保部门在对医疗机构进行评估后，计算出人均医疗费用，按此费用标准向医院预付定额的医疗费，如果实际发生费用超支，超支部分由医院自己承担。

    据了解，不少的专家推崇这一制度，主要是因为该制度较之现行的医保支付方式，加强了对医疗行为的控制，可以有效地遏止医疗机构开大处方、乱检查、乱收费、任意扩大服务项目等弊端，切实减轻患者负担。

    目前我国基本医疗保险费用结算办法按支付方式分主要有三种，即按服务项目付费、按病种付费及总额预付方式，其中大部分地区采用服务项目付费的支付方式。

    正是这种被普遍采用的项目付费，导致现在医疗机构在提供医疗服务过程中，出现了提供过多医疗服务导致费用大幅度上升的情况。而按单病种付费，则由于研究时间较短，此种结算办法主要适用于常见病和多发病，并无法回避鼓励多提供服务项目的激励机制。而总额预付方式则更着重在费用的控制。
